Florida notaries are not authorized to certify copies of birth, death, or marriage certificates. Only the Division of Vital Records/Statistics may perform such a certification. If you need a certified copy of this type of document, you must request it from the state in which the individual was born.  As a fraud prevention measure, many states require that the request form be notarized – a Florida notary can notarize your signature on this form. 

A notary is a public official that the state commissioned to serve as an impartial party to conduct different services. 

•  Administration of affirmations, oaths. acknowledgments
• Stand as a witness during the signing of certain documents

The wording requirements for acknowledgment certificates vary from state to state. 

Some states require specific wording while other states provide Notaries general guidance.  Unlike some states, Florida does not require exact wording, however; the certificate must include elements spelled out in Florida Statutes, 117.05(4), including the venue:  

• The signer's name 
• Type of notarization and date
• Form of identification produced
• Notary's signature, name, and seal
• A statement the signer appeared via the physical presence or online notarization before the Notary.

Florida notaries are not authorized to certify true copies of court-issued documents. Florida notaries ARE authorized to perform marriage ceremonies.

IDENTIFICATION REQUIREMENTS  

A notary is required to exercise a HIGH DEGREE OF CARE in determining the identity of any person whose identity is the subject of a notarial act. Unless such person is known by the notary, identity shall be ascertained by examination of one or more of the following nine unexpired documents. 

• United States Passport
• United States Passport Card
• Certificate of United States citizenship
• Certificate of naturalization
• Foreign passport
• Alien registration card with a photograph
• State-issued driver’s license
• State-issued identification card
• United States military card
• Oath or affirmation of two credible witnesses unaffected by the document or transaction who each personally knows the individual and shows to the notary documentary identification.

You must have an ORI number to request or book Live Scans. If you do not have your ORI number, please contact your employer or requesting agency. All services require valid non-expired government-issued identification (i.e. Driver’s License or Passport). Fingerprints require (2) two forms of identification. There is a 15% processing fee on all credit/debit card refund payment.
